Restoring Your Facial Vitality After Weight Loss
Achieving significant weight loss is an incredible accomplishment, whether through lifestyle changes or with the help of medications like Mounjaro or Ozempic. However, many of my patients are surprised to find that whilst their bodies feel healthier and stronger, their faces can look older, more tired, or deflated. This is an increasingly common concern I'm seeing in clinic, and the good news is that we have excellent treatment options to help restore facial volume, skin quality, and that healthy, vibrant appearance.

What Happens to Your Face During Weight Loss?
When we lose weight, we don't just lose fat from our bodies—we lose it from our faces too. The face has distinct fat compartments that provide structure, contour, and youthfulness. As these deflate, especially with rapid weight loss, several changes occur:
The cheeks can appear hollow or gaunt, the under-eye area may look sunken with prominent dark circles, jowls can become more noticeable as support structures diminish, and the skin itself may look looser or crepey. Because facial fat loss often outpaces the skin's ability to contract and regenerate, you're left with excess skin that has lost its elasticity and underlying support.
This is sometimes referred to as "Ozempic face," though it can happen with any significant weight loss. The face ages not just from lines and wrinkles, but from volume loss and declining skin quality—and rapid weight loss accelerates both of these processes.
The Importance of Skin Quality and Structure
Before jumping straight to replacing lost volume, it's crucial to address the quality and integrity of the skin itself. This is where my approach differs from simply "filling" the face. After weight loss, your skin needs regeneration and strengthening to create a healthy foundation for any volumizing treatments.
Treatment Options: A Strategic Combination Approach
After over a decade of treating post-weight loss concerns, I've found that the most natural, lasting results come from combining treatments that address both skin quality and volume restoration.
Polynucleotides: Cellular Regeneration
Polynucleotides are biostimulators that work at a cellular level to regenerate tissue and improve skin quality from within. They stimulate fibroblast activity, promote collagen and elastin production, and enhance the extracellular matrix. For post-weight loss patients, polynucleotides are invaluable—they help tighten and thicken the skin, improve texture, and restore some of that lost elasticity. This creates a better "canvas" for any subsequent volumizing treatments.
I typically recommend a course of treatments spaced a few weeks apart, with results developing gradually over two to three months as your skin regenerates.
Profhilo and Profhilo Structura: Bio-Remodeling
Profhilo has revolutionized how we approach skin laxity and hydration. This injectable hyaluronic acid treatment spreads beneath the skin to deeply hydrate, stimulate collagen and elastin, and improve overall skin tone and texture. It's particularly effective for the crepey, deflated skin we often see after weight loss.
Profhilo Structura takes this concept further, targeting deeper tissue layers to provide more structural support and lifting. It's especially useful for areas like the cheeks and jawline where more significant support is needed. The combination of surface bio-remodeling with deeper structural enhancement can create remarkable improvements in skin quality and mild lifting without adding excessive volume.
Dermal Fillers: Strategic Volume Replacement
Once we've improved skin quality, strategic dermal filler placement can restore lost facial volume and recreate youthful contours. The key word here is strategic. Post-weight loss faces don't need to be "filled up"—they need intelligent volume replacement in specific areas to restore natural proportions and support structures.
I focus on areas like the cheeks to restore midface support, the temples which often become hollow, the tear troughs to eliminate under-eye shadows, and the jawline to recreate definition. The goal is always natural rejuvenation, not an overfilled appearance. When combined with treatments that have already improved skin quality, fillers integrate beautifully and look completely natural.
PLLA (Poly-L-Lactic Acid): Gradual, Natural Volume Restoration
PLLA, commonly known as Sculptra, is a collagen-stimulating injectable that provides gradual, natural-looking volume restoration. Rather than being a traditional filler, PLLA works by stimulating your own collagen production over time. This makes it ideal for post-weight loss patients who need more substantial volume replacement across larger areas.
Results develop slowly over several months and can last two years or more. PLLA is particularly effective for the cheeks, temples, and overall facial contour. Because it works with your body's natural processes, the results look incredibly natural—people will notice you look healthier and more refreshed, but won't be able to pinpoint exactly what's changed.

Why Combination Treatments Are Essential
Post-weight loss facial rejuvenation isn't a one-treatment solution. The best results come from addressing multiple concerns in a layered, strategic way. A typical treatment plan might include polynucleotides or Profhilo to regenerate and tighten the skin, followed by PLLA for gradual volume restoration in larger areas, and strategic dermal fillers for specific sculpting and support.
This approach creates results that are natural, harmonious, and long-lasting. You'll look like yourself—just healthier, more rested, and revitalized.
Timing Your Treatments
I generally recommend waiting until your weight has stabilized before beginning facial rejuvenation treatments. Continuing to lose weight after treatment can affect your results, and it's difficult to create an effective treatment plan when facial volume is still changing.
Once you've maintained a stable weight for at least three months, we can assess your facial structure and create a personalized treatment plan.
What to Expect
Most of these treatments involve minimal downtime, though some swelling or bruising is possible, particularly with fillers and PLLA. Results develop gradually—this isn't about instant transformation but about creating natural, sustainable rejuvenation that improves over time.
Polynucleotides and Profhilo typically require two to three sessions initially, PLLA usually needs two to four sessions spaced several weeks apart, and fillers provide immediate improvement that settles over a week or two.
